Technical Assistance:
Discussed requirements for ratios and lead teachers in each grouping of children.
Discussed required fall zones for outdoor play equipment.
Reviewed standard 8VAC20-780-420 regarding parental involvement.
Instructed center to contact the health department to request an inspection.

Comments:
An unannounced monitoring inspection was conducted on 6/20/2023 between the hours of 11:40am to 3:45pm. There were 23 children directly supervised by 4 staff. The physical plant, 3 staff records, 4 children?s records, evacuation drills, injury reports, and emergency supplies were inspected. Children were observed participating in a large group activity and eating lunch. Handwashing and naptime were also observed. Areas of non-compliance are identified in the Violation Notice.

Standard #: 8VAC20-770-60-B
Description: Based on record review, the center did not obtain a Sworn Statement of Affirmation prior to the first date of employment for 1 of 3 staff reviewed.

Evidence:
1. Staff #1 began employment on 4/24/2023. The Sworn Statement in the record for Staff #1 was dated 5/8/2023.

Plan of Correction: The center will ensure all new staff complete a sworn statement prior to the first date of employment.

Standard #: 8VAC20-770-60-C-2
Description: Based on record review, 3 of 3 staff records did not contain documentation of Central Registry results by the end of 30th day of employment.

Evidence:
1. Staff #1 began employment on 4/24/2023. The record for Staff #1 did not contain documentation of Central Registry results.
2. Staff #2 began employment on 7/19/2022. The record for Staff #2 did not contain documentation of Central Registry results.

Standard #: 8VAC20-780-160-A
Description: Based on record review, 2 of 3 staff reviewed did not complete a tuberculosis screening within the last 30 calendar days of the date of employment.

Evidence:

1. Staff #1 began employment on 4/24/2023. The tuberculosis screening in the record for Staff #1 was dated 12/12/2022.

2. Staff #2 began employment on 7/19/2022. The tuberculosis screening in the record for Staff #2 was dated 5/12/2022.

Plan of Correction: The center will instruct staff to get a new tuberculosis screening.

Standard #: 8VAC20-780-60-A
Description: Based on record review, 4 of 4 children?s records reviewed did not contain all required information.

Evidence:
1.The record for Child #3 did not contain the first date of attendance.
2. The record for Child #3 did not contain work phone number and place of employment for the father.
3. The record for Child #3 did not contain the address of two designated people to call in an emergency if a parent cannot be reached.
4. The records for Child #1 (start date 4/26/22), Child #2 (start date 3/1/22) and Child #4 ( start date 4/11/22) did not contain documentation of child updates and confirmation of up-to-date information in the child's record as required by 8VAC20-780-420 E 3.

Plan of Correction: The center will follow up with the parents to obtain missing information.

Standard #: 8VAC20-780-70
Description: Based on record review, 2 of 3 staff records reviewed did not contain all required information.

Evidence:
1. Staff #1 began employment on 4/24/2023. The record for Staff #1 did not contain documentation that two or more references were checked before employment.
2. Staff #2 began employment on 7/19/2022. The references in the record for Staff #2 did not contain a date.

Plan of Correction: The center will check their email to find dates of references and document the dates in the record.

Standard #: 8VAC20-780-80-A
Description: Based on record review and staff interview, the center did not maintain a written record of daily attendance documenting the arrival and departure of each child in care as it occurs.

Evidence:

1.There were 23 children in care at the start of the inspection.
2.Staff provided a record of attendance for the previous week and stated the daily attendance had not yet been recorded for 6/20/2023.

Plan of Correction: A staff who is on vacation usually records the attendance. Another staff will record attendance when she is not present.

Standard #: 8VAC20-780-240-A
Description: Based on record review, 2 of 3 staff did not complete the Virginia Department of Education-sponsored orientation course within 90 calendar days of employment.

Evidence:

1. Staff # 2 began employment on 7/19/2022. The Virginia Department of Education-sponsored orientation course in the record for Staff #2 was dated 5/18/2023.

2. Staff #3 began employment on 7/1/2022. The Virginia Department of Education-sponsored orientation course in the record for Staff #3 was dated 5/18/2023.

Plan of Correction: The center will check to see if the training was completed earlier than the date in the record and will ensure all new staff complete the training within 90 days of employment.

Standard #: 8VAC20-780-550-E
Description: Based on record review, Shelter in Place procedures were not practiced a minimum of twice per year.

Evidence:
Records of emergency drills provided during inspection did not contain documentation of shelter in place drills.

Plan of Correction: The center will check with staff who usually documents them to make sure shelter in place drills are being practiced twice per year and documented.

Description: Based on record review, lockdown procedures were not practiced annually.

Evidence:
Records of emergency drills provided during inspection did not contain documentation of a lockdown drill.

Plan of Correction: The center will check with staff who usually documents them to make sure lockdown drills are practiced annually and documented.

Standard #: 8VAC20-780-560-F
Description: Based on observation, the center did not follow the most recent age-appropriate nutritional requirements of the Child and Adult Care Food Program of the United States Department of Agriculture (USDA).

Evidence:
1.Children were served a rotini pasta with minced beef stew for lunch. The lunch served did not include tomatoes and steamed carrots as indicated on the posted menu. The meal did not contain a vegetable, fruit and milk, as required.

Plan of Correction: The center will contact the catering company to ensure lunch requirements are met.
