A complaint investigation was conducted to address an allegation regarding operational responsibilities. The investigation was conducted with the director beginning on June 12, 2023, beginning at 11:00am and ending 11:53am. Staff interview was conducted. Facility documentation was reviewed. The allegations regarding operational responsibilities were determined to be valid.

Standard #: 8VAC20-780-40-E Complaint related: No Description: Based on observation and staff interview, the facility did not ensure that the center maintained compliance with the standards and terms of the current license issued by the department. Evidence: The facility was using the name, Dreamer's Learning Academy, that was not listed on their license issued by the department. Plan of Correction: The facility was approved for a Fictitious Name Certificate from the Commonwealth of Virginia State Corporation Commission which became effective on March 3, 2023. The facility was not aware that this information needed to be provided to licensing prior to using the new name. The facility has applied for a name change to be completed through licensing to come into compliance with the standards.
