Comments:
A self reported complaint was received in the licensing office on April 5, 2023 regarding a seven-year-old child left the classroom unsupervised and unattended in the restroom. On April 6, 2023 an on-site investigation was conducted. Interviews were conducted and observations were made.

Violations:
Standard #: 8VAC20-780-340-A
Description: Based on staff interviews and documentation, the staff supervising children did not ensure their care, protection, and guidance.
Evidence:
1. On April 5, 2023, Child A (7-year-old) left the classroom unattended and went to use the men?s public restroom. It was underdetermined how long the Child A was in the bathroom unattended and unsupervised.
2. At approximately 11:00 am, a community member staff member went to use the public restroom when he noticed Child A was unattended and unsupervised in the restroom.
3. The community staff member notified child day center staff that Child A was in the bathroom.
4. Child A was accompanied back to the classroom by the child day center staff.
5. Based on review of attendance on April 5, 2023, at the time of the incident there were 32 children and 9 staff. Licensing was unable to determine how many children were in the classroom at the time of incident. The center did not have documentation of transitions to different locations.

Plan of Correction: Following the incident on Wednesday, April 5, 2023, at the Sully Community Center with Child A, these corrections have been made.

- Wednesday, April 5, 2023: informed the parents of the incident to bring awareness as well as receive additional insight from the parents about Child A regarding the occurrence.

- Friday, April 7, 2023: purchased and installed chimes/alarms for all four doors entering/exiting the SACC room(s).

- Monday, April 10, 2023: video footage of the chimes/alarms installed on all four doors sent to our site-responsible Licensing Inspector with confirmation of reception.

- Wednesday, April 19, 2023: an HSG active supervision training (see attached handout) was conducted including the staff related to the incident regarding Child A.

- Thursday, May 4, 2023: Updates to various center-specific documents?i.e., Restroom Safety Plan, and Playground Safety Plan (regarding the playground safety scan and community sharing). See attached documents.

- Tuesday, May 23, 2023: an HSG active supervision training catered to center supervisors was conducted to refresh & review the knowledge and skillsets of center supervisors working full-day programs as well as a refresher for school year locations.

- Wednesday, May 31, 2023: an HSG active supervision training catered to the staff working at the Sully Community Center for the summer Camp Fairfax was conducted to educate the summer team of the information about the restroom safety plan, the playground safety plan, as well as field trip safety measures and protocols.

- Wednesday, June 14, 2023: an all-staff training was conducted to review the following:

o Swimming pool supervision safety measures, communication strategies, and positioning.
o Information about children with specific needs to ensure all staff are aware of this information pertaining to children attending our program throughout the summer.

Standard #: 8VAC20-780-340-F
Description: Based on staff interviews and documents, children under 10 years of age should always be within actual sight and sound supervision of staff.
Evidence:
1. On April 5, 2023, Child A (7-year-old) left the classroom unattended and went to use the men?s public restroom. It could not be determined how long the Child A was in the bathroom unattended and unsupervised.
2. Staff acknowledge through written statements and interviews; a child left the classroom unattended. Staff acknowledged the incident occurred between the hours of 11 am and 12 noon.12.

Plan of Correction: Following the incident on Wednesday, April 5, 2023, at the Sully Community Center with Child A, these corrections have been made.
