Comments:
An unannounced monitoring inspection was conducted on June 21, 2023 from approximately 11:00 am to 12:40 pm. Nine staff were present with thirty-nine children ages infants through school age. Children were observed eating lunch and napping. Five children and five staff records were reviewed. Violations were found and are noted on the Violation Notice.

Violations:
Standard #: 22.1-289.011-F
Description: Based on observation, facility did not ensure the most recent inspection findings were posted in a consipicous place on the licensed premises.

Evidence: The most recent inspection findings dated February 2023 were not posted in the facility.

Plan of Correction: The current inspection notices will be posted on the board at the entrance.

Standard #: 8VAC20-780-160-C
Description: Based on review, facility did not ensure 1 of 5 staff have an updated negative TB test or screening dated within the past 2 years.

Evidence: The record for staff#1 (start date 5/12/2008) had a most recent TB screening dated 12/19/2019 which is more than 2 years old.

Plan of Correction: Staff had TB test completed 6/20/2023. Staff will return to get results and results will be placed in staff record.

Description: Based on review, facility did not ensure there is an annual health inspection completed.

Evidence: The most recent health inspection report available for review was dated 10/19/2021.

Plan of Correction: A health inspection was requested again on 6/21/2023.

Standard #: 8VAC20-780-270-A
Description: Based on observation, facility did not ensure areas and equipment of the center are maintained in a clean, safe and operable condition.

Evidence: In the toddler room, a 5" fan was observed on a shelf approximately 5' high. The fan did not have a front protective cover.

Plan of Correction: The fan will be removed from the classroom.

Standard #: 8VAC20-780-340-D
Description: Based on review and interview, facility did not ensure in each grouping of children at least one staff member who meets the qualifications of a program leader or program director is regularly present.

Evidence: In the 4's room, there is not a staff member currently program leader or program director regularly present. The record for staff #5 had documentation of programmatic experience and 14 hours of training for the position. Program Director confirmed the individual is not currently program leader qualified but she is working on completing a training program.

Plan of Correction: Staff will complete the training hours and documentation placed in staff record.
