Technical Assistance:
Staff files need better organization.

You need to establish a system that makes sure all children with allergies/sensitivities/dietary restrictions are listed on the written list and you are obtaining allergy care plans for those children with diagnosed food allergies.

Retraining is needed to make sure staff are documenting all the required components of injuries/accident reports.

The playground equipment should be placed off limits if you are unable to maintain an adequate amount and quality of resilient surfacing. In order to place off limits you want to make it clear that the non compliant area is not being used therefore you should use signage and caution tape to indicate non usage.

There were a number of repeat violations at this inspection. greater attention is needed to make sure you correct those areas immediately.

During the walkthrough the children in the 3 year old classroom were exhibiting some challenging behaviors to include throwing toys and slight roughhousing. Please make sure your teachers in this classroom are addressing the children's needs to include redirecting children to more appropriate activities and if the children are unable to use blocks in a safe manner then they may need to be placed off limits on a temporary basis to ensure they are not being thrown around the room.

Comments:
An unannounced monitoring inspection was conducted today from 9:10 am to 11:20 am with the director, who was present with 58 children and 11 staff. The ages of the children ranged from 8 months to 5 years.The Licensing Inspector was able to observe playtime, handwashing and morning snack. Six children and eight staff files were reviewed as well as 13 injury/accident reports. The center reported that they had no medication on site. The areas of non compliance are outlined on the violation notice.

Description: Based on observation, a review of the allergy/sensitivity/dietary restriction form and interviews with staff, it was determined that the center is not maintaining a written list of all children's allergies/sensitivities/dietary restrictions.
Evidence:
Child B is lactose intolerant and has an egg allergy (which was listed on the child's physical )however the allergy and intolerance were not placed on the written list posted in the classroom.

Description: Based on interview and a review of recordkeeping it was determined that the center has not obtained written allergy care plans on all children with diagnosed food allergies.

Child A has a shellfish allergy and lactose allergy however the center had not obtained allergy care plans for these two allergies.

Description: Based on interview and a review of staff files, it was determined that a staff was missing components of orientation training which is required before being left alone with children or within 7 days of employment.
Evidence:
Staff A was hired on 10/18/22 and was missing components of orientation training to include prevention of sudden death syndrome and use of safe sleep practices, prevention of shaken baby syndrome, abusive head trauma and coping with crying babies and distraught children, recognizing the symptoms of allergic reaction, responding to allergic reaction, preventing exposure to allergies, preventing cross contamination.

Description: Based on observation, it was determined that the center was not maintained in a safe and operable condition.
Evidence:
1.The Licensing Inspector (LI)observed several cracked window panes in the school age classroom. Tape was used on some of the cracks in an attempt to reinforce the windows.
2. The LI observed exposed ground covering on the 3/4/5/schoolage playground. The ground covering presented a tripping hazard.

Description: Based on observation, it was determined that there was insufficient resilient surfacing underneath and surrounding play equipment and inadequate fall zone for outside play equipment.
Evidence:
1.The Licensing Inspector observed on the 3/4/5/schoolage playground a blue/green composite play structure that requires at least 6 inches of resilient surfacing underneath and six feet surrounding the structure.
There was no resilient surfacing underneath and around this play equipment as the mulch that was once there had degraded and turned to dirt and is no longer a resilient surface.
2. There was 2 feet of fall zone for the blue slide on the infant/toddler playground. This slide was within 2 feet of another piece of equipment. This slide required 6 feet of fall zone.

Description: Based on a review of 13 injury/accident reports and interview, it was determined that the center is no documenting all components for injuries/accidents.
Evidence:
