A self report investigation was conducted, beginning March 9 2023 and concluding March 30 2023. The report was regarding a potential violation of forbidden actions. Based off of interviews and inspections the self report was deemed not to be valid.
There were 62 children in the direct care of 15 staff members. A sampling of child records and staffing records were reviewed. The physical plant, evacuation drills, emergency supplies, and the preservice training were reviewed and inspected. Evidence: The diapering pad in the infant room had cracks that made the diapering surface absorbent.Plan of Correction: Not available online. Contact Inspector for more information.
