Technical Assistance:
1. Make sure there is a cover sheet over the allergy/sensitivity/dietary restriction list in the classrooms, the cover sheet provides confidentiality.
2. Make sure to remove all old allergy lists from the classrooms.

Comments:
An unannounced monitoring inspection was conducted today with the director, who was present with 60 children and 8 staff. The inspection was conducted from 1:40 pm to 4:50 pm. The ages of the children present were between 6 months to 5 years. Most of the children were napping during the walkthrough. Six staff and six children's files were reviewed during this inspection.
The fire inspection was dated 2/1/23 and the health inspection was dated 9/20/22.

Violations:
Standard #: 8VAC20-780-280-B
Description: Based on observation, it was determined that hazardous substances were not kept locked.
Evidence:
The Licensing Inspector (LI) observed 3 containers of a cleaning agent, disinfectant wipes and a disinfectant agent unlocked on a shelf in the 2 year-old bathroom.
The LI observed dish detergent and sanitizing spray unlocked in the kitchen.

Standard #: 8VAC20-780-330-B
Description: Based on observation it was determined that there was insufficient resilient surfacing underneath and surrounding the outside play equipment.
Evidence:
On the older playground, the Licensing Inspector observed the following:

1. An exposed tree root measuring 1.5 feet long at the end of the red and green composite climbing structure. The tree root was in the fall zone of the climber and there was no resilient surfacing ( pea gravel) in the fall zone.
2. A half of a foot to the left of the steps to access the tan and green composite climbing structure was bare ground and no resilient surfacing. The standard requires at least 9 inches of pea gravel in the fall zones.

Description: Based on observation and interview it was determined that there was no parental authorization for an over the counter medication for one child.
Evidence:
Hydrocortisone anti itch cream was one site for child A, however there was no parental authorization.

Description: Based on observation it was determined that medication was not kept locked.
Evidence:
The LI observed a tube of hydrocortisone for child A sitting on the table in the classroom. The medication was accessible to the 4/5 year old children in the classroom.
