Technical Assistance:
Technical Assistance given and licensing standard were reviewed regarding the use of blankets, emptying containers with water, staff records, and a defined evacuation plan drawing.

Comments:
An unannounced monitoring inspection was conducted on June 20, 2023. The inspector arrived at 11:25am and departed at approximately 1pm. There were 5 children present and two staff members. A sample size of ten records were reviewed which included five children?s records and five staff records. The Inspector observed staff and children?s interactions throughout the inspection. Children were observed free play, diapering, and nap. Emergency preparedness documents and first aid kits were reviewed. No medications are administered at the center. Transportation is not provided by the center. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented. The violations are listed on the violation notice issued to the center and were reviewed with the site owner at the exit interview.

Violations:
Standard #: 8VAC20-780-140-A
Description: Based on record review and interview, it was determined that the center did not ensure that a physical examination by or under the direction of a physician in is the child's record before the child's attendance or within 30 days after the first day of attendance.
Evidence:
1. Child #1 (date of enrollment 02/06/2023) did not have a physical examination in their record.
2. Child #2 (date of enrollment 04/17/2023) did not have a physical examination in their record.
3. Staff #1 confirmed that there was no physical examination record by or under the direction of a physician for Child #1 and Child #2.

Plan of Correction: The center responded with the following: The Director/Owner will obtain this information immediately from child's parent #1 and #2 and document it in the record.

Standard #: 8VAC20-780-340-C
Description: Based on observation and interview, it was determined that the center failed to ensure that during the stated hours of operation, there always shall be on the premises when one or more children are present one staff member who meets the qualifications of a program leader or program director and an immediately available staff member, volunteer or other employee who is at least 16 years of age, with direct means for communication between the two of them. The volunteer or other employee shall have received instruction in how to contact appropriate authorities if there is an emergency.
Evidence:
1. Inspector arrived at 11:25am with one staff member and 5 children in care. The youngest child was 9 months old. There were no other staff members present in the center.
2. Staff #1 confirmed that they were the only staff member with 5 children.

Plan of Correction: The center responded with the following: The Owner/Director will hire substitute teachers/floaters to fill in when scheduled staff is unable to work their scheduled shift.

Standard #: 8VAC20-780-350-C
Description: Based on observation and interview, it was determined that the center did not ensure that the ratio when in ongoing mixed age groups, the staff-to-children ratio and group size applicable to the youngest child in the group shall apply to the entire group.
Evidence:
1. Inspector arrived at 11:25am with one staff member and 5 children in care. The youngest child was 9 months old and the oldest 7 years old. The staff-to-children ratio shall be 1:4 (staff:children).
2. Staff #1 confirmed that they were the only staff member with 5 children with the youngest child being 9 months old.

Plan of Correction: The center responded with the following: The Owner/Director will ensure that adequate staff is hired to ensure staff-to-children ratio are always met. Interviews for back up staffing is currently in progress.

Standard #: 8VAC20-780-550-G
Description: 8VAC20-780-550. G.
Based on record review and interview, it was determined that the center did not ensure that documentation shall be maintained of emergency evacuation, shelter-inplace, and lockdown.
Evidence:
1. There is no documentation of emergency evacuation, shelter-in-place, and lockdown drills.
2. Staff #1 confirmed completing the drills monthly however there was no documentation of the required recordings of the drills being kept.

Plan of Correction: The center responded with the following: The Director/Owner will ensure that monthly drills are properly documented and maintained according to 8VAC20-780-(7)-550-G.

Standard #: 8VAC20-780-570-E
Description: Based on observation and interview, it was determined that the center did not ensure that prepared infant formula shall be dated and labeled with the child's name.
Evidence:
1. Two bottles in the infant room were not dated and labeled with the child?s name.
2. Staff #1 confirmed that the bottles were not dated and labeled with the child?s name.

Plan of Correction: The center responded with the following: The Director/Owner will ensure that all bottles are labeled with the child's name and date. Written labels has been placed on all bottles.
