Comments:
An unannounced monitoring inspection was conducted today from 9:30 am-11:05 am. There were 37 children (infants-5yrs.) directly supervised by 8 staff. The physical plant, 4 staff records, 4 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group play, reading books, playing outside, and exploring spaghetti worms during an insect lesson. Diapering and hand-washing procedures were also observed. There was an ample supply of materials for the children. Areas of non-compliance are identified in the Violation Notice.

Description: Based upon observation, resilient surfacing was not to depth as required under climbing equipment.

Evidence: There was limited depth of mulch on both playgrounds used by toddlers and preschoolers under climbing equipment and slide areas.

Plan of Correction: We are having mulch donated and are deciding what kind.

Standard #: 8VAC20-780-550-P
Description: Based on record review, written documentation on injuries were missing required information.

Evidence: 3 of 6 Injury reports were missing documentation of method/ date / time parents notified.

Plan of Correction: We will make sure staff fill in required information.
