An unannounced monitoring inspection was conducted today from 9:40 am-12:10pm. There were 42 children directly supervised by 6 staff. The physical plant, 4 staff records, 4 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group play, coloring, math stamping, making paper air planes playing outside and eating snack. Diapering and hand-washing procedures were also observed. There was a n ample supply of materials for the children. Areas of non-compliance are identified in the Violation Notice.

Standard #: 8VAC20-780-270-A Description: Based upon observation, equipment was not maintained in operable condition.
Evidence: 1 tricycle on the playground did not have a pedal.Plan of Correction: Bike was removed during the inspection.
Standard #: 8VAC20-780-550-P Description: Based upon record review, center did not maintain required information while documenting injuries.
Evidence: The date and time parents notified were was not completed on 3/8 forms.Plan of Correction: Will remind staff to complete. The contact was in person when signed by parent. We will have to look at wording on forms.
