An unannounced monitoring inspection was conducted today between the hours of 7:35am and 8:35am. There were 4 children directly supervised by 2 staff. The physical plant, 2 staff records, 2 children?s records, medications, evacuation drills, injury reports, and emergency supplies were inspected. Children were observed eating breakfast and playing outside. Areas of non-compliance are identified in the Violation Notice.

Standard #: 8VAC20-780-140-A Description: Based on record review, the center did not obtain documentation that each child has a physical within 30 days after the first day of attendance.
Evidence:
1. Child #1 began attending the center on
1/19/2022. The record for Child #1 did not
contain documentation of a physical exam.
