Comments:

An unannounced monitoring inspection was conducted on-site June 22, 2023. The director was available during the inspection. There were 30 children present, ranging in ages from 1 months to 10 years, with 11 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 5 staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names; list staff by positions only.

Description:	Based on observation and interview, the center failed to ensure the findings of the most recent inspection were posted in the facility. Evidence: 1. The findings from the most recent inspection conducted March 23, 2023, were not posted in the facility on the date of inspection. 2. Staff #6 confirmed the most recent inspection wasn't posted and stated she threw them away.

Description:	Based on a review of records and interview, the center failed to ensure to obtain a copy of the results of an out-of-state sex offender registry check prior to the first day of employment from any state in which the applicant has resided in the preceding five years. Evidence: 1. The record of staff #3, hired 06/20/2023, did not contain documentation of results of an out-of-state sex offender registry check. 2. Staff #3 identified living in another state in the previous 5 years on the staff's sworn disclosure statement. 3. Staff #6 confirmed the out-of-state sex offender check was not completed.

Description:	Repeat Violation Based on a review of staff records and interview, the center failed to ensure that each staff record reviewed contained a central registry finding within 30 days of employment. Evidence: 1. The record of staff #2, hired 10/07/2022, contained documentation of central registry results dated 06/10/2023. 2. Staff #6 confirmed the record was not followed up on until December 2022.
