An investigation was initiated from 2:00 pm - 3:00 pm to investigate a complaint regarding a forbidden action. Interviews were conducted, documentation, and video footage was reviewed. The complaint regarding forbidden actions is determined to be valid.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Standard #: 8VAC20-780-410-1 Complaint related: Yes Description: Based on interviews and video footage it was determined staff engaged in rough handling which is a forbidden action. Evidence: On 5/11/2023 at around 11:30 am three children can be seen on the carpet playing near a bookshelf. Child A (Age: 21 months) appears to move towards child B (Age: 20 months) in an attempt to bite child B. Staff A can be seen on video pushing child C (Age 23 months) to the side, causing them to fall to the ground, in an attempt to intercept the bite between child A and Child B. Staff A then pulls forcefully on child A causing child A and child B to fall to the floor. Staff A can then be seen swatting child A's arm away. Plan of Correction: Staff A was terminated and all staff were retrained regarding appropriate behavioral guidance methods.
