Comments:
The Licensing Inspector conducted an unannounced inspection on 6/2/2023 in response to a self-reported incident that was reported to the licensing office on 5/31/2023. Interviews were conducted with staff relating to the report of a 15 month old child being left out of sight and sound supervision at the center. The information gathered during this investigation finds the self-report of a child being out of sight and sound supervision as valid. Standard #: 8VAC20-780-40-E
Description: Based on self-reported information, and interview with center staff, the center did not follow their own policies and procedures for accounting for all children when leaving the room.

Evidence: The center did not follow their policy on accounting for all children when both staff in the classroom left a child alone in the classroom for 5 minutes when taking the class to play outside.

Plan of Correction: The center will retrain staff and ensure that all staff follow their policy on safe transitions.

Standard #: 8VAC20-780-340-A
Description: Based on self-reported information, and interview with center staff, the center failed to ensure that when staff are supervising children, they should always ensure their care, protection, and guidance.

Evidence: Neither of the two staff members in the classroom ensured the care, protection, and guidance of Child #1 when the child was left unsupervised in the classroom.

Plan of Correction: The center will retrain staff and ensure that all staff follow their policy on safe transitions.

Standard #: 8VAC20-780-340-F
Description: Based on self-reported information, and interview with center staff, children under 10 years of age were not always within actual sight and sound supervision of staff.

Evidence: Child #1 was left unsupervised for 5 minutes after both staff members in the classroom took the other children to play outside.

Plan of Correction: The center will retrain staff and ensure that all staff follow their policy on safe transitions.
