Technical Assistance:
1. Reviewed labeling requirements of food brought from home.
2. Inspector will provide email address for the central registry.

Comments:
Conducted an unannounced monitoring inspection at 12:03pm. Observed 64 children + 18 direct-care staff. Ratios and supervision were in compliance. The summer camp was in session and children were engaged in a variety of activities to include: story time in Spanish, playing with blocks and puzzles, reading books, washing hands, eating lunch, and preparing for naptime. Staff were actively engaged with the children and activities. Children bring their own lunch from home daily. The facility was found to be clean and well supplied toys and equipment for the children. Areas of non-compliance with standards and laws reviewed were found. Questions about this inspection may be directed to pamela.sneed@doe.virginia.gov

Standard #: 22.1-289.011-F
Description: Based on observations made, staff failed to ensure that the most recently issued violation notice, dated 10/18/22, was posted in the school. Evidence: A Violation Notice dated 12/6/21 was posted on the bulletin board.

Plan of Correction: We have displayed the violation notice in the kitchen bulletin area, and we will also ensure it is posted in the front door bulletin area.

Standard #: 22.1-289.035-B-4
Description: Based on records reviewed, staff failed to ensure that 1 of 5 staff records included documentation of completed out-of-state background checks within the 1st 30-days of employment. Evidence: Staff #8 - Began employment 12/12/22 and to-date there is no documentation of a completed out-of-state child abuse and neglect background search/central registry.

Plan of Correction: Our HR department will immediately contact the Maryland background check agency to follow-up on the request that was initially sent on 4/03/2023. Additionally, we will resend the request to ensure its expedited processing.

Standard #: 8VAC20-780-70
Description: Based on records reviewed, staff failed to ensure that 2 of 5 staff records included required documentation.

Evidence:

1. Staff #8 - The education documents needed to qualify the staff for the position they are working were not translated into English.
2. Staff #12 - 0 of 2 reference checks were documented.

Plan of Correction: The HR department has sent the documents for translation and has received the translated document, which approves the bachelor's degree of the lead teacher.

Two reference letters have been located in our DC office and have been added to the staff file in Falls Church.

Standard #: 8VAC20-780-240-A
Description: Based on records reviewed, staff failed to ensure that 1 of 5 staff records included documentation of the VDOE sponsored orientation course (10 hours). Evidence: Staff #8-Employment began 12/12/22 and to-date there was no documentation of the 10-hour course being completed.

Plan of Correction: The staff have already submitted their certification for the 10-hour orientation training required by VDOE.

Description: Based on interview conducted, the licensee failed to ensure that a required emergency supply item was on-site. Evidence: There was not a working battery-operated radio in the school.

Plan of Correction: We have purchased a battery-operated radio per request.

Standard #: 8VAC20-780-550-P
Description: Based on records reviewed, staff failed to ensure that written records of children's injuries were documented as required.

Evidence:

1. 4 of 4 written injury reports did not document the time the parent was notified of the child's injury.
2. 2 of 4 written injury reports did not document how the parent was notified of the child's injury.

Plan of Correction: All staff members have been notified in person and through written reminders, emphasizing the importance of writing the day and the way parents are notified.

Standard #: 8VAC20-780-560-G
Description: Based on observations made, staff failed to ensure that food brought from home was labeled with the child's name and date. Evidence: Multiple lunch bags in 2 classrooms did not include the date.

Plan of Correction: We have personally informed all staff members and sent written reminders emphasizing the daily requirement of labeling the lunch boxes with the name and date.
