Comments:
A non-mandated inspection was conducted on June 26, 2023 from 10:50 A.M.-12:00 P.M., in response to a serious injury report received in the Office of Child Health and Safety (Valley) on June 23, 2023. The inspector reviewed compliance in the areas of administration, staffing and supervision, programming, and special care and emergencies. A total of one child record and three staff records were reviewed, four interviews were conducted, classroom observation conducted, and review of injury reports and documentation were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-780-340-A
Description: Based on review of documentation and interviews, the center failed to ensure that when staff are supervising children, they shall always ensure their care and protection.

Evidence:
1. On 06/21/2023 in the Toddler Classroom, (children 2 years of age), Orbeez Water beads were used in the sensory table between 3:30 P.M.-4:00 P.M.
2. A two year old child placed the water beads in one of her nostrils and one of her ear canals. The child required outside medical care to remove the water beads.
3. The center was notified by the parents on 06/21/2023 at 6:15 p.m. that they were on route to the emergency room due to the water beads being in the child's nostril and ear canal.
4. The recommended age for Orbeez Water beads is for children five years of age and older.
5. Staff #1, #2, and #3 verified in interviews and documentation that the water beads were placed in the sensory bin on 06/20/23 and 06/21/2023 to used with children two years of age.

Plan of Correction: When playing with sensory items, the staff have been asked to increase supervision by monitoring the children in smaller groups and rotating activities. 6/22/2023
Administration will increase classroom inspections for hazardous materials, checking classrooms 1x a week using a checklist beginning 7/1/2023

Description: Based on observation, interviews, and review of documentation, the center failed to ensure that all materials available shall be age and stage appropriate for the age of the children.

Evidence:
1. On 06/21/2023 in the Toddler Classroom, (children 2 years of age), Orbeez Water beads were used in the sensory table between 3:30 P.M.-4:00 P.M.
2. The recommended age for Orbeez Water beads is for children five years of age and older.
3. Staff #1, #2, and #3 verified in interviews and documentation that the water beads were placed in the sensory bin on 06/20/23 and 06/21/2023 to used with children two years of age.

Plan of Correction: All water beads were removed from the classroom immediately 06/22/2023. Administration inspected classrooms for any hazardous materials and verified that sensory items were age appropriate. As of 6/26/2023, all staff were informed that any outside materials must be approved before being taken into the classroom. During the July 11th staff meeting, classroom materials will be discussed and an acknowledgment form will be provided for Staff to sign.
