Comments:
An unannounced complaint inspection was conducted to investigate an allegation regarding supervision of children and procedures for emergencies. Interviews were conducted, documentation was reviewed and observations were made. The portion of the complaint regarding supervision was determined to be valid. The portion of the complaint regarding procedures for emergencies was determined to be not valid. Violations were issued as a result of this inspection.

Standard #: 8VAC20-780-80-B-2
Complaint related: No
Description: Based on review of documentation and interview, the facility failed to inform the department's representative as soon as practicable, but not to exceed two business days, of any injury to a child that occurs while the child is under the supervision of the facility and requires outside medical attention. Evidence: On 6/5/2023, between approximately 5:00-5:30pm, child #1, age five, sustained a head injury outside on the playground. The child was taken by the parent to the emergency room on 6/6/23 and was diagnosed with a concussion. The parent informed the center director, who then completed a report for the company, but the facility did not report this injury to the department.

Plan of Correction: Moving forward, the director will ensure to call and email licensing to report as well as submit the IROMT.

Description: Based on review of documentation, and interviews, the facility did not ensure that when staff are supervising children, they ensure the care and protection of all children. Evidence: on 6/5/2023, between approximately 5:00-5:30pm, child #1, age five, was crying when they approached staff #1 and stated that they had been pushed by another child. Staff asked the child if they were ok, but did not conduct an assessment to determine what happened, how and where it happened, or whether or where the child had sustained an injury. Staff stated they went and talked to the group of children and allowed the child to continue playing. The child had sustained a red mark, approximately 2 inches long, on the left side of their head above the ear, and was diagnosed with a concussion the next day.

Plan of Correction: The day the incident occurred on 6/5/23 the director had already left for the day. On 6/6/23 the director met with the child's mother and immediately went to speak with the teacher. The director retrained the teacher on assessing a child when an injury is reported.

Complaint related: No
Description: Based on staff interview, and review of documentation, the facility failed to have at least one staff in each classroom or area where children are present with current certification in CPR and first aid. Evidence: on 6/5/2023, between approximately 5:00-5:30pm, staff #1 was alone outside on the playground supervising approximately four children in the JK class. The record for staff #1 did not have documentation of CPR and first aid training.

Plan of Correction: Staff #1 completed the online portion of FA/CPR and has been scheduled for a FA/CPR hand on portion for July 19, 2023.
