Comments:
An unannounced monitoring inspection was conducted today between the hours of 10:30am and 1pm. 32 children were observed directly supervised by 5 staff. The physical plant, 4 staff records, 4 children?s records, evacuation drills, injury reports, and emergency supplies were inspected. Children were observed engaged in imaginative play, completing puzzles and making crafts. The children were learning about a featured artist as part of their summer camp program. There was an assortment of age appropriate materials for the children in care. The center was clean and organized. Areas of non-compliance are identified in the Violation Notice.

Standard #: 8VAC20-780-50-B
Description: Based on record review and staff interview, the center did not maintain documentation as required for 1 of 4 children's records.

Evidence:
1. The immunizations in the record for Child #3 were dated 2/1/2023, after their first date of attendance (1/2/2023).
2. Staff stated the original immunizations obtained for Child #3 before the child was admitted were destroyed once a more up to date document was obtained. Staff stated they have been removing old immunization forms from the files when parents bring updated documents.

Plan of Correction: Moving forward, all documents will be maintained in the record for no less than 2 years after termination of services.
