Comments:
A monitoring inspection was completed from 12:00 until 2:25 in response to a self-reported incident concerning supervision of a child. A mandated monitoring inspection was also conducted. Interviews were held and one child's record was reviewed. On child's record and one staff record was reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-780-340-F
Description: Based on interviews, the center failed to always have within actual sight and sound supervision of staff children under 10 years of age.
Evidence:
1. On 04/05/23 at 5:30 p.m. child #1 (17 mo. old) was observed by staff #1 with a bottle of liquid pain over the counter medication in her hand. Staff #2 removed the bottle from the child's hand.

Plan of Correction: Emergency personnel were notified and came to the center to evaluate the child and it was determined that the child was not affected. Parents of the child were immediately notified of incident. Parent of the diaper bag that the child got the medication from was contacted and the parent reported there was one dose left in the bottle. ALL parents were notified via email reminding them that no medications were to be left in diaper bags. Shelves are being evaluated to be moved to be placed higher in the room. Plastic bags have been ordered for families to use to bring necessary times to the center.
