Comments:
A Subsidy Health and Safety Inspection was conducted on Thursday, June 15, 2023 to review supplemental health and safety requirements. The inspector was on site from 9:00am to approximately 2:05pm. There were a total of 130 children present in the direct care of 15 staff members. The director and assistant director assisted the inspector throughout the inspection. Upon the inspector's arrival, the children and staff were observed in their respective classrooms. The children were observed during transitions, during free-play, and teacher-led activities. The center is equipped with age-appropriate materials and equipment for the children's use. Staff were engaged with the children and offered guidance when needed. The areas where children receive care were inspected and found to be in compliance. The required postings were observed. Transportation is provided and vehicles used for transportation were inspected. Medication is administered and medications and authorizations were reviewed. During the inspection, ten children's records and ten staff records were reviewed.

A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Information gathered during the inspections determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-790-600-B
Description: Based on a review of ten staff records and interview, the vendor did not ensure one staff completed the Virginia Preservice Training for Child Care Staff within the first 90 days of employment.

Evidence: 1) The record of Staff #1, employed 06/01/21, did not contain documentation that the staff completed the Virginia Preservice Training for Child Care Staff. 2) During interview, a member of management confirmed the Virginia Preservice Training for Staff #1 could not be located.

Subsidy vendor staff that were hired prior to 10/13/2021 were required to complete the Virginia Preservice Training for Child Care Staff within the first 90 days of employment.

Plan of Correction: The training will be completed and filed.

Standard #: 8VAC20-790-600-D-1
Description: Based on a review of ten staff records and interview, the vendor did not ensure four staff members had current certification in cardiopulmonary resuscitation (CPR) appropriate to the ages of children in care within 90 days of the date of employment.

Evidence: 1) The record of Staff #1, employed on 06/01/21, did not contain documentation that the staff has current certification in CPR.

2) The record of Staff #7, employed on 08/22/22, did not contain documentation that the staff has current certification in CPR.

3) The record of Staff #8, employed on 11/01/22, did not contain documentation that the staff has current certification in CPR.

4) The record of Staff #10, employed on 02/13/23, did not contain documentation that the staff has current certification in CPR.

5) During interview, a member of management reported the center does not have documentation that the staff have current certification in CPR.

Plan of Correction: A CPR/First aid class will be scheduled for staff to complete as soon as possible.

Description: Based on a review of ten staff records and interview, the vendor did not ensure four staff members had current certification in first aid appropriate to the ages of children in care within 90 days of the date of employment.

Evidence: 1) The record of Staff #1, employed on 06/01/21, did not contain documentation that the staff has current certification in first aid.

2) The record of Staff #7, employed on 08/22/22, did not contain documentation that the staff has current certification in first aid.

3) The record of Staff #8, employed on 11/01/22, did not contain documentation that the staff has current certification in first aid.

Description: Based on a review of ten staff records and interview, the vendor did not ensure three staff members who work directly with children completed the department's health and safety update course annually.

Evidence: 1) The most recent health and safety update course in the record of Staff #1, employed on 06/01/21, was completed on 03/23/21. The health and safety update course was not completed annually.

2) The record of Staff #2, employed on 04/25/22, did not contain the department's health and safety update course. The course should have been completed no later than 04/25/23.

3) The record of Staff #4, employed on 06/06/22, did not contain the department's health and safety update course. The course should have been completed no later than 06/06/23.

4) During interview, a member of management reported the documentation that staff completed the course could not be located.

Plan of Correction: The training will be completed annually moving forward.
