An investigation in response to a self reported incident was initiated on June 29, 2023 and concluded on June 29, 2023. The report received by the department included information of a staff member that left a child out of sight and sound supervision and areas of the center that were not clean and operable.
Staff interviews were conducted, staff and children records were reviewed, and the physical plant was inspected.
The evidence gathered during the investigation supported the self-report of non-compliance with standards or law, and violations were issued. Any violations not related to the self report but identified during the course of the investigation can be found on the violation notice. Standard #: 8VAC20-780-40-E Description: Based on staff interviews and documentation review, the licensee did not ensure that the center's own policies and procedures on face to name procedure.
Evidence:
1. On 06/22/2023 at approximately 12:00pm, Staff #1 was transitioning the preschool class from the hallway bathrooms to the Big Gym for naptime. Staff #1 opened the door to the Big Gym and the children went to their nap mats. Staff #1 reopened the door to the Big Gym and found Child #1(3 years old) standing alone on the other side of the door. Child #1 was left alone for approximately one minute.
2. According to Face To Name Procedure document, Face to Name Transition Sheets must be used to document staff to child ratios at the following times throughout the day: Transitioning children from one location to another.
3. Staff #3 confirmed that staff members should conduct Face to Name before crossing every threshold.
4. Staff #1 did not conduct a Face to Name check before entering the Big Gym.Plan of Correction: All staff will be retrained on Face to Name procedures.
Standard #: 8VAC20-780-270-A Description: Based on staff interviews and documentation review, areas and equipment of the center, inside and outside, were not maintained in a clean, safe and operable condition.
Evidence:
1. Five of the toilets in the hallway that the preschool classroom use were not flushed and clean. Three of the toilets contained urine, feces, and toilet paper and two contained black marks around the toilet.
2. Staff #1 and Staff #3 confirmed that the bathroom toilets were not clean and that the toilets did not flush properly.Plan of Correction: Work orders were placed to fix the flushing issues with the toilets. The situation has been rectified and the center will ensure the health and safety of the children.
Standard #: 8VAC20-780-340-F Description: Based on staff interviews and documentation review, children under 10 years of age were not within sight and sound supervision of staff.
Evidence:
1. According to an Incident Report, on 06/22/2023 at approximately 12:00pm, Staff #1 was transitioning the preschool classroom from the hallway bathrooms to the Big Gym for nap time. Staff #1 opened the door to the Big Gym as children went to nap mats. Staff #1 reopened the Big Gym door and found Child #1 standing on the other side of the closed door. Child #1 was alone for approximately one minute.
2. Staff #1 and Staff #2 confirmed that Child #1 was left out of sight and sound supervision for one minute.Plan of Correction: Staff is no longer employed. Staff will be retrained on Face to Name procedure.
