Comments:
A monitoring inspection was conducted today, May 1, 2023. The licensing inspector was onsite from approximately 2:00pm until 3:50pm. The director was on site and participated with the inspection. There were 23 children present, ranging in ages from seven months to seven years, with four staff supervising. Children were observed having snack and later playing in centers that consisted of blocks, coloring and dinosaurs. Infants and toddlers had playtime on the floor and in the exersuacer. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, and medication. A total of four children's records and four staff members? records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and a violation was documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-780-60-A-8
Description: Based on review of four children?s records, the center did not have a written allergy action care plan for one child diagnosed with a food allergy, to include instructions from a physician regarding the steps to take in the event of an allergic reaction.

Evidence:

The record for Child #3 (enrolled 01/12/2022) indicated the child has a diagnosed food allergy. A written food allergy action plan had not been obtained as of 05/01/2023

Plan of Correction: Per the director: We will ask the parent to provide the allergy care plan.

Standard #: 8VAC20-780-270-A
Description: Based on observations and interview, the center did not maintain all areas and equipment in a clean, safe, and operable condition.

Evidence:

A protruding nail and split board were observed as part of the playground fencing creating components that could entangle clothing or snag skin.

Plan of Correction: Per the director: I will have my husband repair the fence.

Standard #: 8VAC20-780-530-A-1
Description: Based on review of five staff records, the center did not ensure at least one staff in each classroom or area where children were present had current cardiopulmonary resuscitation (CPR).

Evidence:

Staff #5 was observed supervising eight children in the Two?s Classroom. Staff #5 was the only staff member supervising the eight children in the classroom. Staff #5 did not have current CPR as of 05/01/2023.

Plan of Correction: Per the director: We will place staff with current CPR in each classroom and have staff without CPR certification schedule a class to obtain it.

Description: Based on review of five staff records, the center did not ensure at least one staff in each classroom or area where children were present had current certification in first aid.

Evidence:

Staff #5 was observed supervising eight children in the Two?s Classroom. Staff #5 was the only staff member supervising the eight children in the classroom. Staff #5 did not have current certification in first aid as of 05/01/2023.
