Comments:

A monitoring inspection was conducted on May 22, 2023. The inspector was on site from approximately 3:05 pm until 4:45 pm. The site director was present and assisted with the inspection. There were 28 children present, ranging in ages from six years to twelve years, with two staff supervising. Children were observed playing in centers that consisted of painting, blocks, and board games. Later children had a team meeting and talked about respect. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of five child records were reviewed. Three staff records were review virtually on May 23, 2023. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Standard #:	22.1-289.035-B-4
Description:	Based on review of three staff records, the center did not request an out of state search of the child abuse and neglect registry as required for each state an individual has resided in the past five years within 30 days of employment. Evidence: On 3/29/2023, the center was issued a violation notice citing an out-of-state child abuse and neglect registry check for staff #2 (employed 09/13/2022) that had not been requested by the 30th day of employment. On 03/29/2023 the center was notified of the violation and required to provide documentation within 10 days of the notification that an out-of-state child abuse & neglect registry had been requested. On 05/23/2023, a follow-up review of the record of staff #2 (employed 09/13/2022) did not contain documentation of a request for the out-of-state child abuse and neglect registry. This is a repeat violation from 03/29/2023.
