Technical Assistance:
A percentage of all staff files were reviewed. Each staff person's emergency contact information is requested be kept also on file within their internal computer file system. It was previously discussed that emergency contact information be primarily kept on site and will be reviewed during in person inspections

Comments:
The following inspection took place between the hours of 10 am through 3:30 pm. Staff files were reviewed by a representative from the Commonwealth of Virginia, Department of Education, Office of Child Care Health and Safety (Fairfax) and through the County of Loudoun, Department of Parks, Recreation, and Community Services main office located in Leesburg, Virginia. A total of 8 staff files were reviewed for this facility.

Information gathered during the inspection determined that there were (4) non-compliances with the applicable standards, and code. These violations were documented on the Violation Notice issued to the center. Thank you for your cooperation during this inspection. Violations:
Standard #: 22.1-289.035-A
Description: Based on review, 2 of 8 staff files indicated that not all background checks were repeated every five years.

Evidence -
1. On the date of inspection (06/27/2023), the following staff had expired central registry findings on file: Staff #7 (dated - 03/19/2018), and Staff #8 (dated - 10/23/2017).
2. This non-compliance was observed after a review of each staff person's digital file.

Plan of Correction: These 2 staff's central registry search requests were submitted and paid for on 6/30/23. Once the results are received, they will be uploaded into their respective digital staff records.

Standard #: 8VAC20-780-160-A
Description: Based on review, 1 of 8 staff files indicated that the facility has not ensured that each staff member has submitted documentation of a negative tuberculosis screening.

Evidence:
1. On the date of inspection (06/27/2023), there was no documentation on file of a negative tuberculosis screening for staff #2.
2. The start date for Staff #2 was listed as 11/15/2005.

Plan of Correction: Staff #2 is a customer service rep and was unaware of this requirement. She completed this on 6/27/23.

Standard #: 8VAC20-780-70
Description: Based on review, 1 of 8 required records were not kept for each staff member.

Evidence:
1. On the date of inspection (06/27/2023), there was no documentation of staff #6 completing orientation.
2. The start date for Staff #6 was listed as 05/25/2022.

Plan of Correction: The document saved as orientation for staff #6 did not include all required information. The correct documentation will be uploaded into her digital staff record.

Standard #: 8VAC20-780-240-A
Description: Based on review, 1 of 8 staff files indicated that the Virginia Department of Education sponsored orientation course was not completed within 90 calendar days of employment.

Evidence -
1. On the date of inspection (06/27/2023), documentation of the VA Pre-service 10-hour training was not available for review for Staff #3.
2. The start date for Staff #3 was listed as 11/29/2022.

Plan of Correction: Staff #3 will complete the 10 hour Preservice training.
