Technical Assistance:
A percentage of all staff files were reviewed. Each staff person's emergency contact information is requested be kept also on file within their internal computer file system. It was previously discussed that emergency contact information be primarily kept on site and will be reviewed during in person inspections

Comments:
The following inspection took place between the hours of 10 am through 3:30 pm. Staff files were reviewed by a representative from the Commonwealth of Virginia, Department of Education, Office of Child Care Health and Safety (Fairfax) and through the County of Loudoun, Department of Parks, Recreation, and Community Services main office located in Leesburg, Virginia. A total of 10 staff files were reviewed for this facility.

Information gathered during the inspection determined that there were (2) non-compliances with the applicable standards, and code. These violations were documented on the Violation Notice issued to the center. Thank you for your cooperation during this inspection. Violations:
Standard #: 8VAC20-780-160-C
Description: Based on review, 1 of 10 staff files indicated that it has been more than 2 years from the date of last tuberculosis (TB) documentation kept on file.

Evidence -
1. On the date of inspection (06/27/2023), the TB documentation on file for Staff #8 was dated - 03/25/2021.
2. A more current TB documentation was not available at the time of inspection.

Plan of Correction: Staff #8 will have this completed.

Standard #: 8VAC20-780-70
Description: Based on review, 1 of 10 required records were not kept for each staff member.

Evidence:
1. On the date of inspection (06/27/2023), there was no documentation of staff #10 completing orientation.
2. The start date for Staff #10 was listed as 09/16/2021.

Plan of Correction: Staff #10 will attend a Licensing Orientation training when she returns from LWOP.
