Comments:
A renewal inspection was conducted on July 3, 2023 from 10:28 A.M.-1:15 P.M. There were 29 children present, ranging in ages from two years to twelve years of age, with seven staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of five child records, seven staff records, and one agent record were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 22.1-289.035-B-4
Description: REPEAT

Based on a review of staff files, the center failed to ensure that all out-of-state-background checks were completed within the correct timeframe.

Evidence:
1. Staff #1 indicated on the sworn disclosure statement dated 06/16/2023 that they had lived outside of Virginia within the past five years.
2. There was not an out of state sex offender name check on file. Staff #1's first day of employment. was 06/26/2023.
3. Staff #2 verified that there was not an out of state sex offender name check completed.

Plan of Correction: Administration completed the sex offender name check while the Licensing Inspector was on site. In the future all out of state sex offender name checks will be completed prior to the first day of employment.

Standard #: 8VAC20-780-280-B
Description: REPEAT

Based on observation, the center failed to ensure that hazardous materials were kept in a locked location.

Evidence:
1. In the Adventurer's restroom on a shelf above the toilet there was a spray bottle labeled bleach water spray and one bottle labeled sanitizer.
2. In the Explorer's restroom in a basket on the shelf above the toilet there was a container of Clorox Disinfectant Wipes. On the front of the container it stated "Caution", and "Keep out of reach of children."

Plan of Correction: Administration will remind all staff the requirements for all hazardous materials to be locked. Administration will complete weekly checks of all classrooms to ensure that hazardous materials are kept locked.

Standard #: 8VAC20-780-550-P
Description: Based on review of injury reports, the center failed to ensure all required information was on the injury reports.

Evidence:
1. An injury report dated 04/13/2023 did not contain future action to prevent reoccurrence of the injury, and there was only one signature out of the two required.
2. An injury report dated 04/14/2023 did not contain the date the parent was notified.
3. An injury report dated 04/24/2023 did not contain the following information: future action to prevent reoccurrence of the injury, the date the parent was notified, how the parent was notified, the time the parent was notified, and two signatures.

Plan of Correction: During a staff meeting all staff will be retrained on how to complete an injury report. Administration will review injury reports as they are completed to ensure all required information is on the report.

Standard #: 8VAC20-780-560-G
Description: Based on observation, the center failed to ensure that food brought from home was clearly dated.

Evidence:
1. In the Adventurer's classroom there were four lunchboxes that were not dated.
2. In the Explorer's Classroom there was a snack box on the table that was not dated, and two lunchboxes on the hooks that were not dated.
3. In the Trailblazer cubbie room there was a lunchbox setting on the floor that was not dated.
4. In the School Age Classroom there were two lunchboxes on the hooks that were not dated.

Plan of Correction: Administration will contact parents and request they place the date on the lunchboxes. Upon arrival staff will ensure the date is on the box before placing on the cubbie hooks.
