Comments:
A complaint investigation was conducted on 6/2/2023. The complaint alleged children's injuries as a result of a lack of supervision, lack of injury reporting to parents, inadequate diaper changing procedures to ensure proper hygiene. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, and special care and emergencies. The investigation included review of documentation, interviews and observation.


Information gathered during the investigation did not provide evidence to substantiate the allegations of a lack of supervision. injury reporting or hygiene practices/diapering. However, the investigation did reveal non-compliance with an applicable complaint-related standard and a violation was documented on the violation notice issued to the program.

Description: Based upon staff interview, the facility has not ensured that the operational responsibilities of the licensee include ensuring that the center's activities are maintained in compliance with the center's own policies and procedures regarding consistency of care.
Evidence:
The facility's written policy regarding consistency of care/primary caregiving indicates that children "will be assigned a primary caregiver" so a child is able "to form an immediate and stable relationship with a caregiver".
a. Interviews of staff revealed that the three staff persons (staff 1, 2 and 3) on duty during the day in the two year old classroom were in their third week of working together in the classroom during the week of May 22, 2023 - May 26, 2023. Staff stated that the children were not clearly divided into groups (with a primary caregiver) until the week of May 29, 2023.
b. Staff stated that staff 4, program leader of the two year old classroom, had been moved to another classroom, after which the staff in the two year old classroom had not been assigned primary caregiver roles.

Plan of Correction: The facility responded with the following:
Staff in the two's classroom were retrained on primary care groups on 5/24/2023. Each teacher in the room is assigned to a group of children. This is displayed on a white board in the classroom. As students move into or out of the classroom and or show toilet training readiness the primary care groups are reevaluated to meet the need of the children and the classroom. Management will do random checks in the classroom to ensure primary care groups are being used properly.
