Comments:
An unannounced renewal inspection was conducted on 07/07/2023. There were 45 children, ages 19 months- 11 years, and 8 staff members present during the inspection. The inspector reviewed compliance in the areas of administration, staff qualifications and trainings, staffing and supervision, physical plant, programs, emergencies and nutrition. The children were observed during free choice play, participating in circle time, having a morning snack, in outside large motor play, and lunch. A total of 5 children?s records, 5 staff records, 1 applicant record, and 3 medications were reviewed. The inspector arrived for the inspection at 8:50 am and departed at 11:40 am.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.

Violations:
Standard #: 8VAC20-780-270-A
Description: Based on observation, the center failed to ensure that areas and equipment, inside and outside, shall be maintained in a clean, safe and operable condition.

Evidence:
1. The rest cots were stacked on top of each other in the school-age classroom. The stack of cots was unsecured and tilted out away from the wall creating a potential for the stack of cots to fall over. The stack of cots was in an area where children were playing and walking.
2. There was a plastic picnic table outside that was broken. The table had cracks and missing pieces that created pinch points and sharp edges that could snag skin.
3. There was a tricycle on the playground that had a broken seat with sharp edges.

Plan of Correction: Will get them corrected.

Standard #: 8VAC20-780-510-G
Description: Based on medication review, the center failed to ensure that medication shall be labeled with the child?s name, the name of the medication, the dosage amount, and the time or times to be given.

Evidence: Medication 3 was in a zip lock bag and did not have a label with the child?s name, the dosage amount, and the times to be given.

Plan of Correction: Will get a label.
