Comments:
An unannounced renewal inspection was conducted today between the hours of 8:30am-1:30pm. There were a total of 82 children with 12 supervising staff members present. Children were observed during breakfast, handwashing, centers, engaged in group activities and outdoor play time. A selection of staff and children records, medications and other documents were reviewed. The physical space, emergency supplies, evacuation drills, attendance records, and the Emergency Preparedness Plan were reviewed. Areas of non-compliance are identified in the violation notice. Violations:
Standard #: 8VAC20-780-160-A
Description: Based on record review, each staff member did not submit documentation of a negative tuberculosis screening within the last 30 calendar days of the date of employment.
Evidence:
1. Staff #5's record (Date of hire-05/04/23) had documentation of a negative tuberculosis screening dated 01/16/23.

Plan of Correction: Moving forward, management will ensure that TB screenings will be within 30 calendar days of the first date of employment.

Standard #: 8VAC20-780-270-A
Description: Based on observation, areas and equipment, inside and outside, shall be maintained in a clean, safe and operable condition.
Evidence:
1. In the Toddler Room, there is peeling and chipped paint along the wall in the sensory area and the carpet is unraveling along the edges.
2. In the School Age classroom, the seam of the carpet has unraveled in the library area.
3. In the Preschool classroom, the carpet is unraveling along the edges in the home living area.
4. In the Early Preschool classroom, the rug located in the block area has unraveled exposing the fibers of the carpet.
5. In the Two's classroom, there is chipped paint and an inch and a half hole located in the wall near the block area of the classroom.

Plan of Correction: Management will submit a ticket and attach inspection report to the Facilities Team to correct the peeling paint. New carpets will be ordered for each classroom.

Description: Based on observation, cosmetics, medications, or other harmful agents were stored in areas, purses or pockets that were accessible to children,
Evidence:
1. In the Infant classroom, a purse was on the ground under the high chairs that contained deodorant and cosmetics.

Plan of Correction: The staff member will be retrained on locking up personal items.

Standard #: 8VAC20-780-330-B
Description: Based on observation of the playground, the minimum of 6 inches of resilient surface material (mulch) was not present under the slide and climber on the large playground.
Evidence:
1. The mulch located at the bottom of the fall zones on the playground was not maintained in such a manner that it complies with the minimum safety standards of the depth of six inches.

Plan of Correction: Management will submit a ticket to the Facilities Team to order mulch for the playground.

Standard #: 8VAC20-780-350-B-2
Description: Based on observation, and staff interview, staff to ratios were not maintained.
Evidence:
1. In the two year old classroom, there was total of 12 children with two supervising staff. The youngest child in the classroom was under the age of two years old; therefore lowering the ratio to 1:5.

Plan of Correction: The child will be placed into the toddler classroom until the child turns the appropriate age for the classroom.

Standard #: 8VAC20-780-350-C
Description: Based on observation and staff interview, staff to children ratio and group size in ongoing mixed groups were not applicable to the youngest child in the group.
Evidence:
1. The Early Preschool classroom has a mixed age group of 2 1/2 and 3 year old children. The Early Preschool classroom contained a total of nine children with one supervising staff member. The ratio for the classroom is 1:8.The youngest child in the classroom was 2 1/2 years old; therefore exceeding the 1:8 ratio requirement.
2. Staff #6 confirmed that the Early Preschool classroom maintains a 1:8 ratio.

Plan of Correction: Management will review procedures and be retrained for reporting going over ratio in a classroom.

Standard #: 8VAC20-780-500-A
Description: Based on observation, children's hands were not washed with soap and running water or disposable wipes before and after eating meals or snacks.
Evidence:
1. In the infant classroom, Child #6(14 months) hands were not washed before eating breakfast.

Plan of Correction: The staff member will be retrained on handwashing procedures.
