Technical Assistance:
Discussed the importance of transitions during lunch time and ensuring that adequate supervision is provided to all children who are eating and drinking.

Discussed the documentation requirements to ensure children are adequately immunized. A religious/medical exemption form will be forwarded as a resource.

Discussed the importance of training staff to seek assistance when needed when working with children with behaviors and special needs.

Use best practice during the hot summer days and ensuring the playground equipment's surface is not harmful.

Comments:
An unannounced Monitoring inspection was conducted today. A total of 41 children with 18 staff were present. A walk through of all 7 classrooms was made, staff interviews conducted and observation of programming was made. As discussed, each classroom could benefit from updating and augmenting some of the materials and equipment. Outdoor equipment and playground areas were also observed. Please be mindful of the summer elements and maintaining the fencing and equipment during this time. Only one medication need identified. The school age children were on a field trip to the water park. Children were observed coloring, playing outside and transitioning to lunch. Lunch today was cheese sticks with marinara, fruit and broccoli with milk to drink. Some food allergies identified and based on staff interviews, they presented knowledgeable of the nutritional needs in their classrooms. A total of 5 children's records, and 6 staff files were reviewed. Lead qualifications were verified with the exception of Classroom Intermediate A that is currently seeking a lead teacher. Standard #: 8VAC20-780-130-E
Description: Based on child record review, additional immunizations every six months were not obtained for 1 child under the age of two years.
Evidence:
Child #5 is under the age of 2 years. The last documentation of immunizations was dated 08/2022.

Plan of Correction: Family was emailed today 7/11/23 for child updated vaccine records. Parent emailed back that they
have contacted their doctors and are working to get this submitted asap. As the new school year is
approaching we will be auditing all children shot records and collecting updates.

Standard #: 8VAC20-780-40-E
Description: Based on review of the center's own electronic system, staff failed to ensure the center's own policies and procedures were followed.
Evidence:
1. The center's policy on classroom transitions requires that staff enter the location of each child onto an electronic system to ensure that the location of each child is known.
2. Staff did not enter a transition and the current location for 5 out of 15 children in the PreK classroom.

Plan of Correction: Will review transition protocols with all staff. All teachers doing transitions will be observed for the next
60 days and feedback provided. After 60 days transitions will be spot checked.

Standard #: 8VAC20-780-240-I
Description: Based on review of staff training, documentation of orientation training was incomplete.
Evidence:
The documentation of orientation training for Staff # 1 did not include the training delivery method and the individual providing the training.

Plan of Correction: Going forward will ensure the new hire orientation training is signed by the person conducting the training. Will go back and sign this orientation to be in compliance.

Standard #: 8VAC20-780-270-A
Description: Based on observation of the Intermediate playground, a piece of equipment was not maintained in a safe and operable condition.
Evidence:
A plastic push toy had areas that were cracked and sharp. This toy is no longer operable and was found on top of a water activity table accessible to children in care.

Plan of Correction: We immediately disposed of this piece of equipment while licensing was here

Standard #: 8VAC20-780-340-F
Description: Based on observation, staff failed to provide intermittent sight supervision for one child in the restroom.
Evidence:
1. The layout of the bathroom between the PreK and the Intermediate B classroom has doors leading from the bathroom to each classroom.
2. Both doors leading from the bathroom to each classroom were left open.
3. Child #1 was observed. by the Principal, leaving the bathroom and entering the Intermediate B classroom which was unoccupied during this time.
4. The 2 staff in the PreK classroom did not provide intermittent sight supervision of Child #1 while Child #1 was in the bathroom.

Plan of Correction: Review and retrain with all teachers to ensure that children are under proper supervision when using
the restrooms and provide some best practices to help support the teachers.

Standard #: 8VAC20-780-570-B
Description: Based on observation of the Toddler B classroom, a bottle was used in a child's designated sleeping location.
Evidence:
One child was observed laying down on their cot drinking an infant bottle in the Toddler B classroom.

Plan of Correction: Will retrain all infant and toddlers? teachers (including assistant teachers that help
out in the classrooms) on the topic of children with bottles. Children must be always held by a teacher
or seated when drinking their bottles, they can?t be laying down or walking.
