Comments:
A renewal inspection was conducted on 07/11/2023 from approximately 10:23 a.m. to 12:50 p.m. and 1:10 p.m. to 2:00 p.m. There were 45 school age children present and 6 staff supervising. The inspector reviewed compliance in the areas of administration, qualifications and training, physical plant, staffing and supervision, programming, medication, special care and emergencies, and nutrition. Five child records and five staff records were reviewed.
The information gathered during the inspection determined violations with applicable standards or law. A violation notice was issued. Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today, 07/24/2023. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s). Description: Based on child record review, the provider failed to ensure that three of five immunizations records for children were signed by a physician, designee, or official of a local health department.
Evidence:
The immunization records for C2, C4, and C5 were not signed by a physician, designee, or official of a local health department.

Based on child record review, the provider failed to ensure that one of five records contain written agreements between the parent and the center as required by 8VAC20-780-90.
Evidence:
The record for C1 did not document the authorization for emergency medical care, the statement that the parent will pick up a sick child as soon as possible when requested by the center, and the statement that the parents will inform the center within 24 hours if the child or member of the immediate family has developed any reportable communicable disease.

Description: Based on observation and interview, provider failed to maintain staff-to-child ratios as required by 8VAC2-780-350.B.5.
Evidence:
Licensing inspector observed 20 children with one staff outside on the playing field. The children were school age eligible through 12 years old. The required staff-to-child ratio for this age group is 1 staff to 18 children. The program director acknowledged the group was out of ratio.

Description: Based on observation, the tables used for eating were not sanitized before use.
Evidence:
Licensing inspector did not observe tables being cleaned and sanitized before the children ate their lunch. The tables were used for a photography activity immediately prior to lunch being served.
