Comments:
The licensing inspector conducted an unannounced inspection on Tuesday, July 11, 2023 in response to a self-reported incident that the Department received on July 6, 2023 regarding supervision. The inspector was on site at the center on July 11, 2023 from 11:50am to 12:45pm. Interviews with staff were completed, observations were made in the center, and documentation was reviewed.
The preponderance of the evidence gathered during the investigation validates the self-reported incident. A violation was cited regarding supervision. See the violation notice for more details.

Violations:
Standard #: 8VAC20-780-340-A
Description: Based on interviews conducted on July 11, 2023, the center failed to ensure care and protection for an infant in care who received another child's formula.
Evidence:
1. The center's representative and staff #1 reported the following: On 07/6/2023, staff #1 fed child #1 four ounces of formula belonging to child #2. The interviews also revealed that child #1 and child #2 do not drink the same infant formula.
2. When referring to child #1 and child #2 during an interview on July 11, 2023 staff #1 stated, ?They look alike and it was a bad mistake on my part.?

Plan of Correction: The Director has retrained the staff on safe bottle feeding. Making sure that name to face call to co-workers in the infant room and getting a confirmation back from that staff member before feeding the bottle to the child. This safe bottle feeding will continue going forward and management will observe to make sure it is being completed.
