22.1 EARLY CHILDHOOD CARE AND EDUCATION

Comments:
A monitoring inspection was conducted today, July 17, 2023, as a follow-up to the issuance of a provisional license. The inspector was on site from approximately 11:35 am until 12:50 pm. The
director was present and assisted with the inspection. There were 29 children present, ranging in ages from 13 months to 10 years, with seven staff supervising. Children were observed preparing for lunch, having lunch and listening to a story. The inspector reviewed compliance in the areas previously cited to include administration and special care and emergencies.
Background checks for five staff were reviewed. The inspector interacted with staff and children in care.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 8VAC20-780-550-P
Description: Based on review of seven written records of children?s serious and minor injuries, the center did not ensure the record contained all required documentation.

Evidence:

1. Incident report #1 for child #1 (dated 06/20/2023) did not contain the time and date the parent was notified of the injury, and how the parent was notified.
2. Incident report #2 for child #2 (dated 05/10/2023) did not contain the time and date the parent was notified of the injury, and how the parent was notified.
3. Incident report #3 for child #3 (dated 04/21/2023) did not contain the date the parent was notified of the injury.
4. Incident report #4 for child #4 (dated 03/08/2023) did not contain the time and date the parent was notified of the injury, and how the parent was notified.
5. Incident report #5 for child #5 (dated 03/06/2023) did not contain the staff member who was present, the time and date the parent was notified of the injury, and how the parent was notified.
6. Incident reports #6 and #7, both for child #6 (dated 02/15/2023 and 02/22.2023) did not contain the staff present, the time and date the parent was notified of the injuries, and how the parent was notified.

Plan of Correction: Per the provider: I will have staff use the state's injury form rather than the procare program that doesn't include all the required elements.
