An unannounced inspection in response to a self-reported incident that occurred on March 17, 2023 regarding an allegation of forbidden action by staff. The inspection was initiated on March 20, 2023 and completed on May 31, 2023. The inspector was on site on April 13, 2023 interviewing staff, conducting observations at the center, and reviewing documentation.
The preponderance of the evidence gathered during the investigation does not validate the self-reported incident.
The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
