Comments:
An unannounced monitoring inspection was conducted on 07/13/2023. There were 20 children, ages 2 years - 12 years, and 5 staff members present during the inspection. The inspector reviewed compliance in the areas of administration, staff qualifications and trainings, staffing and supervision, physical plant, programs, emergencies, nutrition and background checks. The children were observed during self-directed activities, outdoor time and mealtime. The inspector discussed the following with the provider: measurement for capacity, outdoor equipment, and infant standards. A total of 5 children?s records, and 5 staff records were reviewed. The inspector arrived for the inspection at 10:00 am and departed at 12:15 pm.

The information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice to the provider.

Violations:
Standard #: 8VAC20-780-70
Description: Based on record review and interviews with staff, the center failed to ensure that staff records contain all of the elements as required by the standards.

Evidence:
1. The record for Staff 1 documented a hire date of 06/14/2023. The record for Staff 1 did not contain the following: documentation that two or more references as to character and reputation as well as competency were checked before employment. Staff 2 confirmed this information was not available.
2. The record for Staff 5 documented a hire date of 09/21/2023. The record for Staff 5 did not contain the following: documentation that two or more references as to character and reputation as well as competency were checked before employment. Staff 2 confirmed this information was not available.

Plan of Correction: Staff will obtain information before hire.

Standard #: 8VAC20-780-80-A
Description: Based on observation, the center failed to ensure that for each group of children, the center shall maintain a written record of daily attendance that documents the arrival and departure of each child in care as it occurs.

Evidence: The attendance record in the school age room indicated 10 children present. There were 13 children in care at the time the attendance record was reviewed. The attendance did not document the arrival as it occurred.

Plan of Correction: Staff will document arrival and departure of children as it occurs.

Standard #: 8VAC20-780-520-A
Description: If diaper ointment or cream is used, these products do not need to be kept locked but shall be inaccessible to children.

Evidence: There were 2 tubes of diaper ointment on the second shelf of the changing table in the 2- 4-year-old room. It was accessible to children.

Plan of Correction: The ointment was moved during the inspection and will be inaccessible to children.
