Comments:
An unannounced monitoring inspection was completed from 10:30am until noon. There were 7 children present with three staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 3 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 8VAC20-780-330-B
Description: Based on observation and
measurement, the resilient surfacing
did not meet the required depth of 6
inches of resilient surfacing.
Evidence:
1. The mulch in the fall zones of the
blue curved slide , the smaller blue
slide and the yellow climber
measurement approximately 2-4
inches. The required depth is 6 inches.

Plan of Correction: The director will arrange to have additional mulch added to the fall zones to meet the required depth.
