Comments:

Today an unannounced Monitoring Inspection took place between the hours of 11:55am and 2:00pm. There was 1 classroom observed with a total of 7 children with 2 staff. The children were observed playing freely, eating lunch and transitioning to nap. A complete inspection of the physical plant, children and staff records, fire drill log and emergency procedures were observed during this inspection. Violations were found during today?s inspection and can be viewed on the Violation Notice. Thank you for your cooperation during the inspection. Standard #:	8VAC20-780-50-A
Description:	Based on observation, records were not treated confidentially. Evidence: The Supplemental page of the most recent inspection was observed posted on the bulletin board inside the front door. The Supplemental page clearly states "DO NOT POST" and "confidential".
Plan of Correction:	I will double check what documents are being posted.
Standard #:	8VAC20-780-60-A
Description:	Based on record review and interview, each center shall maintain and keep at the center a separate record for each child enrolled. Evidence: Staff #1 stated that 7 out of 7 children enrolled at the center are also enrolled at another center owned by the staff. Staff #1 confirmed that they photo copied all children's records from the first location and created another record for each child at the second location. The centers are not owned by the same legal entity and records cannot be transferred.
Plan of Correction:	I'll re-do all the forms and have the parents review to make sure the information is correct.
Standard #:	8VAC20-780-80-A
Description:	Based on a review of daily attendance, the center did not document the arrival and departure of each child in care as it occurs. Evidence: At the start of the inspection there were 6 children documented on the centers attendance sheet. There were 7 children in attendance. Child #2 was not documented as being present.
Plan of Correction:	I'll make sure that all parents are signing in.
Standard #:	8VAC20-780-350-B-2
Description:	Based on observation, the center did not follow a 1:5 ratio for children between 16 months up to 24 months. Evidence: At the start of the inspection, Child #2 (23 months) was observed in a group with 7 children supervised by 1 staff.
Plan of Correction:	I'll make sure to have enough staff.
Standard #:	8VAC20-780-380-A-2
Description:	Based on observation, all children were not presented with an opportunity to rest for at least 30 minutes prior to being given quiet activities. Evidence: There was no cot available for Child #5 to rest. Child #5 was observed sitting at a desk with a book.
Plan of Correction:	She doesn't sleep so that's why she's at the desk. I'll get a cot out for her.
Standard #:	8VAC20-780-440-B
Description:	Based on observation, cots and cribs were not identified for use by a specific child. Evidence: 5 cots and 1 crib were observed being used. Cots and cribs were not labeled for each specific child.
Plan of Correction:	I'll label the cots.
Standard #:	8VAC20-780-530-C
Description:	Based on observation, there were not at least two staff members with current first aid certification and CPR certification on the premises during the center's hours of operation when children are in care. Evidence: At the start of the inspection (approximately 11:55am) Staff #2 was observed supervising 7 children alone. Staff #1 arrived at the center at approximately 12:14pm.
Plan of Correction:	I'll make sure to do food delivery.
Standard #:	8VAC20-780-560-F
Description:	Based on observation, the center did not follow the most recent, age-appropriate nutritional requirements of the Child and Adult Care Food Program of the United States Department of Agriculture (USDA). Evidence: The children were observed being served cheese pizza, fruit cups and milk for lunch. The children were not served a vegetable.
Plan of Correction:	Sometimes we order them lunch in the summer. We'll make sure that we have fruit, vegetables and milk available.
