Comments:
An unannounced renewal inspection was conducted on July 11, 2023 from approximately 12:15 pm to 2:15 pm. There were 56 children in attendance and a total of 9 staff present. 3 staff records and 6 children?s records were viewed. All areas of the center were observed including classrooms, bathrooms, and the playgrounds. The children were observed playing outside, playing games on the carpet, and taking a nap while the licensing inspector was on-site.

Violations were cited as a result of this inspection.

Please complete the plan of correction and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today, 07/21/2023. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again, and 3) person(s) responsible for implementing each step and/or monitoring any preventative measure(s).

Standard #: 8VAC20-780-160-C
Description: Based on records review, and interview, the center failed to ensure that each staff member shall submit documentation of a negative tuberculosis screening at least every two years from the date of the initial screening or testing. Evidence: The record for staff #1, date of employment 01/01/01, contained a tuberculosis screening dated 01/20/21. A repeat screening was due on 01/20/23.

Standard #: 8VAC20-780-510-P
Description: Based on documentation review, and observation, the center failed to obtain an updated medication authorization when the medication authorization expires.
Evidence:
Documentation review revealed that the medication authorization for child #5, date of enrollment 11/07/22, expired 04/2023. When an authorization for medication expires, the parent shall be notified that the medication needs to be picked up within 14 days or the parent must renew the authorization. Medications that are not picked up by the parent within 14 days will be disposed of by the center by either dissolving the medication down the sink or flushing it down the toilet. The medication was observed in the medication lock box.
