Technical Assistance:
Technical assistance was offered related to the following:
1. Infant bottles
2. Attendance
3. Classroom management to ensure privacy and safety for children during transitions
4. Environmental Sanitation Report
5. Center's policy related to abusive head trauma

Comments:
A renewal inspection was conducted on 7/24/23 from 10:35 am to 1:30pm. There were 96 children present in direct care and supervision of 16 staff present. The children were observed engaged in water play activities, diapering, free play in the playroom and eating lunch. The lunch menu included BBQ beef patty, slider buns, cauliflower, pineapples and milk. The outdoor play area was also assessed for compliance. A total of 4 children's records, 6 staff records, attendance, accident report logs, medications and emergency drill records were reviewed.

Violations:
Standard #: 8VAC20-780-270-A
Description: Based on observation, equipment used inside the center was not maintained in a safe and operable condition.
Evidence: In the Infant C classroom, two soft cushioned pillows had covers that were torn. The inner soft cushion material was exposed to the children in care.

Plan of Correction: Removed torn items from the classroom; replacements for the item will be purchased, if needed.

Standard #: 8VAC20-780-280-B
Description: Based on observation, hazardous substances such as cleaning agents were not kept inaccessible to children.
Evidence: In the Preschool 3-2's classroom, a bottle of bleach and water solution was found sitting on the sink between the bathrooms. It was accessible to children in care.

Plan of Correction: Retrain employees on proper methods of locking cleaning agents and other hazardous substances when not in use.

Standard #: 8VAC20-780-500-A
Description: Based on observation, center staff did not ensure that staff and children's hands were washed after toileting.
Evidence:
1. In the toddler room, Staff # 1 was observed changing diapers. Staff # 1 changed two children's diapers and did not ensure their hands were washed with warm running water and soap.
2. Staff # 1 was observed changing disposable gloves between each diaper change. Staff # 1 did not wash her hands with warm running water and soap between the two diaper changes.

Plan of Correction: Retrain employees on proper handwashing procedures during diapering, including between each diaper change, regardless of use of disposable gloves.

Standard #: 8VAC20-780-500-B
Description: Based on observation, staff did not follow the proper cleaning and sanitizing procedures as required by the licensing regulations.
Evidence:
1. In the toddler room, Staff # 1 was observed changing diapers.
2. Staff # 1 did not use the soap and water solution to clean the diaper pad before using the bleach solution.

Plan of Correction: Retrain employees on proper diapering procedures, including cleaning and sanitizing between each diaper change using soap/water spray and then a bleach/water solution.

Standard #: 8VAC20-780-560-G
Description: Based on observation, food brought from home was not labeled with each child's full name and date.
Evidence:
1. It was observed in the Preschool 3-1's and Preschool 3-2's classroom that children were eating snack brought from. The snacks were not labeled with their full name and date.
2. 4 infant bottles were found in the Infant A classroom that were not dated for today.

Plan of Correction: Parents were notified regarding labeling all food, bottles, and home-snacks with the child's full name and today's date.
