An investigation was initiated on June 21, 2023, and concluded on July 18, 2023 in response to a self-reported allegation received by the department on June 16, 2023 regarding supervision. The inspector conducted an onsite unannounced inspection on June 21, 2023, from approximately 10:10am until 2:30pm. Interviews with staff were completed, observations were made at the center, and documentation was reviewed. Additional interviews were conducted on July 13, 2023. The preponderance of evidence gathered during the investigation does support the self-reported allegation; therefore, the allegation was determined to be valid.
