Standard #: 22.1-289.035-B-4 Description: Based on record review and discussion with staff, the center failed to obtain a copy of the criminal record check and a sex offender registry check from any state in which the individual has resided in the preceding five years.
Evidence: The record for Staff 3 contained documentation that Staff 3 had resided outside of the state of Virginia within the preceding five years. The record for Staff 3 contained a documented hire date of 08/16/2021. There was not a criminal record check from all of the states in which the individual had resided in the preceding five years available for review during the 07/25/2023 inspection.Plan of Correction: Will get the form completed and sent off to complete the request.
Standard #: 8VAC20-780-160-A Description: Based on record review, the center failed to ensure that each staff member shall submit documentation of a negative tuberculosis screening at the time of employment or prior to coming into contact with children and that the documentation shall have been completed within the last 30 calendar days of the date of employment.
Evidence:
1. The record for Staff 1 contained a documented hire date of 06/14/023. The record for Staff 1 did not contain documentation of a negative tuberculosis screening; the tuberculosis screening was not obtained at the time of employment or prior to coming into contact with children.
2. The record for Staff 5 contained a documented hire date of 01/09/2023. The tuberculosis screening in the record for Staff 5 was dated 09/09/2022; the tuberculosis screening had not been completed within the last 30 calendar days of the date of employment.Plan of Correction: Will request today.
Standard #: 8VAC20-780-60-A Description: REPEAT VIOLATION
Based on record review, the center failed to ensure that the separate record for each child enrolled shall contain all of the elements as required by the standards.
Evidence:
1. The record for Child 1 did not contain documentation of viewing proof of the child?s identity and age.
2. The record for Child 2 did not contain the name, address, and phone number of two designated people to call in an emergency if a parent cannot be reached (the address was incomplete for the 2nd designated person listed)Plan of Correction: The information was requested during the inspection.
Standard #: 8VAC20-780-70 Description: Based on record review, the center failed to ensure that staff records contain all of the elements as required by the standards.
Evidence: The record for Staff 3 did not contain the following information: documentation that two or more references as to character and reputation as well as competency were checked before employment; and documentation to demonstrate that the individual possesses the orientation training.Plan of Correction: Will get the references today and will get the training completed.
