An unannounced complaint inspection was conducted in response to a complaint that was received on April 28, 2023. The center staff and parents were interviewed, the child was observed, and video footage was reviewed relating to allegations involving supervision of children. The preponderance of the evidence gathered during the investigation supports the allegation; therefore, the complaint is determined to be valid. A violation notice was issued. Please complete the ?plan of correction? and ?date to be corrected? for the violation cited on the violation notice and return it to me with your signature within 5 business days from today, 08/15/2023. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s). Standard #: 8VAC20-780-340-F Complaint related: Yes Description: Based on interviews and review of video, the center failed to ensure that children under 10 years of age always shall be within actual sight and sound supervision of staff.
Evidence:
Child, C1, age 4, was observed on video footage leaving a classroom and exiting the center without being noticed on 04/27/2023. Video shows C1 walking to the back room that is connected to the Big Kids? classroom. C1 was then seen opening the backdoor of the center, walking down a sidewalk to stairs that lead to a busy shopping center and the driveway of a government office.
C1 was out of the classroom for approximately 2 minutes before the two staff in the classroom realized he was missing. C1?s mother arrived at the center approximately two minutes after the teachers realized C1 was missing, and she assisted in looking for C1. The video showed a local government employee walking to the center. Then, the local government employee and the child?s mother leaving the center going toward the local government office together. Based on interviews with the local government employee and the child?s mother, C1 was reunited with his parent at the local government office.Plan of Correction: Not available online.
