Comments:
An unannounced non-mandated inspection, as required by the Conditional License, was conducted on 7/25/2023 for Champion Ealy Learning Center. There were 18 children, ages 5 months - 10 years, 1 administrative staff and 2 staff present during the inspection. The inspector reviewed compliance in the areas of administration, personnel, environment and equipment, care of children, preventing the spread of disease, emergencies and nutrition. A total of 4 children?s records and 1 staff record were reviewed. The children were observed during creative play, restroom break and craft time. The inspector arrived for the inspection at 8:50am and departed the center at 10:40am.

Discussion on standards related to sight and sound supervision. Discussion on standards related to staff records.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.

Violations:
Standard #: 8VAC20-770-60-C-2
Description: Based on review of one staff record, the center failed to ensure that all background checks were complete.

Evidence: Staff #1 was hired on 6/12/2023 and on the day of the inspection did not have the required central registry results by the 30th day of employment documented.

Plan of Correction: Provider will obtain central registry results.

Standard #: 8VAC20-780-270-A
Description: Based on observation, the center failed to ensure that areas and equipment, inside and outside, shall be maintained in a clean, safe and operable condition.

Evidence: The side wall in the infant room has a phone jack next to an outlet that has pulled away from the wall exposing a hole approximately 1 ?? in diameter and wiring connected to the phone jack. Children were observed in the area and it is accessible to children.

Plan of Correction: Provider will remount phone jack to wall closing exposed wiring and hole.

Standard #: 8VAC20-780-440-B
Description: Based on observation and discussion with staff, the center failed to ensure that all cribs, cots, rest mats or beds shall be identified for use by a specific child.

Evidence: The 3 cribs and 5 cots located in the infant room were not labeled with the name of the specific child assigned to.

Plan of Correction: Provider will label cribs and cots with children's names assigned to.

Standard #: 8VAC20-780-560-G
Description: Based on observation and discussion with staff, the center failed to ensure when food is brought from home the food container shall be sealed and clearly dated and labeled in a way that identifies the owner.

Evidence: Children?s lunch boxes were observed in the after school classroom and on the day of the inspection were not labeled with the date and the child?s name.

Plan of Correction: Provider will label lunch boxes with names dates of children.
