Technical Assistance:
1. Reviewed the return of expired medications to parents requirements.

Comments:
Conducted an unannounced monitoring inspection at 10:40am. Observed 21 children + 7 direct-care staff. Ratios and supervision were in compliance. Children were engaged in a variety of activities to include: counting, art, playing with slime the children made the day prior, active outdoor play, and one class was having a "Sesame Street Day." Staff were actively engaged with the children and activities. Classrooms were found to be clean and sufficiently supplied toys and equipment for the children. Art made by the children decorated the classrooms and hallways. Areas of non-compliance with standards and laws reviewed were found. Violations:
Standard #: 22.1-289.035-B-4
Description: Based on records reviewed, the licensee failed to ensure that 1 of 4 staff records included and out-of-state background check within the first 30-days of employment. Evidence: Staff #8 began working 3/1/23 and to-date does not have a completed out-of-state child abuse and neglect search on-file.

Plan of Correction: we followed up with MD again (3rd try) to see what happened to the background check-waiting for them? ASAP [sic]

Standard #: 8VAC20-780-70
Description: Based on records reviewed, the licensee failed to ensure that 2 of 4 staff records included documentation of the education needed to qualify them for the positions they are working.

Evidence:

1. Staff #4 - The education diploma was not on-file. Staff #4 began working 10/28/22.
2. Staff #6 - The education diploma was not on-file. Staff #4 began working 5/1/23.

Plan of Correction: Staff member #6 sent in diploma immediately when we asked #6 7.24 Staff Member #4 has been reminded (AGAIN) that this is imperative. She is having trouble getting a copy from her HS #4 ASAP [sic]

Standard #: 8VAC20-780-280-B
Description: Based on observations made, staff failed to ensure that a hazardous substance wad kept in a locked place that prevents access by children. Evidence: A spray disinfectant was sitting on a shelf and accessible to children in a 3-year-old classroom.

Plan of Correction: bottle was put away during the visit. All teachers have been reminded [sic]
