Comments:
An unannounced monitoring inspection was initiated on 7/13/23 with the local department of social services and law enforcement to investigate a self report received by the department related to care, protection and guidance. Based on evidence gathered through interviews and documentation review, the self report could not be substantiated, however, there were violations issued as a result of this inspection. An exit interview was conducted on 7/27/2023.

Please complete the columns for "Plan of Correction" and "Date to be Corrected" for each violation cited on the violation notice, and then return a signed and dated copy to the licensing office by 4:00pm on Friday, 8/4/2023. Violations:
Standard #: 8VAC20-780-340-D
Description: Based on interviews and documentation review, the facility failed to ensure that in each grouping of children at least one staff member who meets the qualifications of a program leader shall be regularly present. Evidence: on 6/29/23 at approximately 3:30pm, staff #2 was left alone with four children in the infant plus one classroom. Staff #2 is listed as an assistant teacher and does not have program leader qualifications documented in their record.

Standard #: 8VAC20-780-350-Q
Description: Based on interviews and documentation review, the facility casually reassigned a child from their regular group and staff disrupting the child's schedule and attachment to their staff members and group. Evidence: On 6/29/23 at approximately 3:30pm, an infant was moved from the Infant plus one classroom to the infant classroom in order to reduce the ratio to allow a staff member to go home.

Description: Based on observation, not all handwashing procedures were followed as required. Evidence: two infants were observed drinking a bottle and their hands were not washed afterwards.

Description: Based on observation, the facility failed to ensure that high chairs, infant carrier seats or feeding tables are used for children under 12 months who are not held while being fed. Evidence: two infants in the infant A classroom were observed drinking bottles; one was laying on the floor with their head on a staff member's leg and the other was laying on a boppy pillow.
