An unannounced monitoring inspection was conducted on June 29, 2023, in response to a self-reported incident that was received on June 14, 2023 in reference to behavior guidance. Interviews were conducted and documentation was reviewed. The preponderance of evidence gathered during the investigation does not support violation of standards regarding the self-report. Additional violations, not related to the self-report, were cited.
Please complete the plan of correction and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today, 07/28/2023. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again, and 3) person(s) responsible for implementing each step and/or monitoring any preventative measure(s).

Standard #: 22.1-289.035-B-2 Description: Based on records review, and interview, the center failed to ensure that the results of a criminal history records check are received prior to the first day of employment. Evidence: The record for staff #2, date of employment 05/08/2023, contained the results of a criminal history records check dated 05/16/23.
