Comments:
An investigation was initiated from 1:00 pm - 2:00 pm to investigate a complaint regarding administration and procedures for emergencies. Interviews were conducted, documentation, child records, and staff records were reviewed. The portion of the complaint regarding administration was determined to be valid. There was not a preponderance of evidence to substantiate the portion of the complaint regarding procedures for emergencies therefore this portion of the complaint is determined to be not valid.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Complaint related: Yes
Description: Based on interviews it was determined that the center failed to inform the department's representative within two business days of any injury to a child that occurs while under the
supervision of the center and requires outside medical attention. Evidence: On 06/30/2023 Child A (age 3 years) sustained an injury to their eyebrow in the preschool classroom that required outside medical attention. This incident was not reported by the facility to the office of child care health and safety.
