Comments:

A renewal inspection was conducted on 07/28/2023. A tour of the facility was conducted, in addition to a record review and interviews with staff. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:

Standard #:	22.1-289.035-B-4
Description:	Based on a record review the center did not ensure all staff who have lived outside of the state of Virginia within the last 5 years, had the required background checks from that state. Evidence: Staff #1, hired 07/03/2023, did not have documentation of having a criminal record check, central registry check or sex offender check from the state that was indicated on the signed sworn statement of affirmation. The director confirmed that the request for the out of state background checks had not been requested yet.
Plan of Correction:	The steps that will be taken to correct this matter are by making sure any person employed from out of the state has their background check completed before they start at our center. This process is to be implemented and monitored by myself the Center Director. The employee in question is no longer employed at the center therefore this has been corrected as of 7/28/2023, her last day of employment here.
Standard #:	8VAC20-780-60-A
Description:	Based on a record review the center did not ensure all required information was kept for each enrolled child. Evidence: Child #1 (enrolled 03/18/2023) did have documentation that staff viewed proof of identification.
Plan of Correction:	The steps that will be taken to correct this matter are by making sure any child enrolled in the center has a complete registration process prior to their start with us. This process is to be implemented and monitored by myself the Center Director. The child in question is no longer at the center therefore this has been corrected as of 7/28/2023, her last day here
Standard #:	8VAC20-780-560-F
Description:	Based on observation and record review, the center did not ensure all requirements were met when choosing to provide meals. Evidence: The menu posted in the center's lobby listed turkey tacos as the lunch to be served to the children that day in care. Children were observed during lunch eating grilled chicken sandwiches.
Plan of Correction:	The steps that will be taken to correct this matter is by making sure that if any changes are made to the menu, we make note of it on the menu posted in the bulletin and that it is communicated to the parents as well. This process is to be implemented and monitored by myself the Center Director. This will be implemented every time there is a change to our school menu.
