A non-mandated monitoring inspection was conducted on July 27, 2023 from 11:00 A.M.-12:30 P.M. There were 60 children present, ranging in ages from four months to five years of age, with nine staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, special care and emergencies and nutrition. A total of three child records and three staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 8VAC20-780-140-A Description: Based on review of children records, the center failed to obtain a physical examination before the child's first date of attendance, or within 30 days after the first day of attendance.
Evidence:
1. Child #3's first date of attendance was 03/01/2023. The physical on file was dated 04/18/2023.
2. Staff #1 verified the date on the physical form.Plan of Correction: We kept pushing the parent to bring the form in and asked them to keep the child home until the form was completed.
Standard #: 8VAC20-780-500-A Description: REPEAT VIOLATION:
Based on observation, the children and staff's hands were not washed at all the required times.
Evidence:
1. In the Infant Room,Staff #2 did not wash her hands before putting on gloves to complete a diaper change.
2. In the Infant Room, Staff #2, did not wash the child's hands with soap. She took the infant to the sink and ran the child's hands under warm water.Plan of Correction: The director and assistant director reviewed handwashing policy for both employees and children with staff. Staff had to sign an acknowledgment memo to be kept on file.
Standard #: 8VAC20-780-570-B Description: Based on observation, the center failed to ensure that bottles were not used while the child is in their designated sleeping location.
Evidence:
1. On July 27, 2023 when the Licensing Inspector entered the Infant Classroom, an infant was observed in their crib drinking their bottle.
2. The crib was designated as the child's designated sleeping area.
3. Staff #2 verified that the child has a bottle in their designated sleeping area.Plan of Correction: Held a staff meeting with all employees and reviewed that absolutely no bottle feeding in designated sleep areas.
