Comments:
A complaint was received in the licensing office on 7/12/2023 regarding release of children?s records. On 7/17/2023 an on-site investigation was conducted. Interviews were conducted and records were reviewed. At the time of this investigation, the preponderance of evidence supported the allegation and the complaint is deemed valid. Description: Based on staff interview, children's records were not made available to parents on request, unless otherwise ordered by the court.

Evidence:
1. On 6/29/2023, an email request for Child A's attendance records was made. A follow-up email was sent on 7/8/2023 asking for the attendance records to be sent by 7/10/2023. The facility did not respond to the request and did not give the parent the records after additional attempts by the parent (by phone and email) were made to follow-up on the initial email. As of the time of the inspection, the records had still not been sent.

Plan of Correction: The requested attendance records were emailed to the parent on the same day that the complaint investigation was conducted. Moving forward, we will send child records to parents as requested.
